1. ACCEPTANCE OF TERMS

By accessing our website or requesting transportation services from Healthy Heart Mobility (“Company,” “we,” “our,” or “us”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree, you must not use our services.

2. DESCRIPTION OF SERVICES

Healthy Heart Mobility provides non-emergency medical transportation (NEMT) services, including but not limited to:

check_circleDialysis, rehabilitation, and clinic visits

check_circleWheelchair and mobility-assisted transport

check_circleScheduled and recurring rides

infoWe do not provide emergency medical services.

3. ELIGIBILITY

To use our services, you must:

Be at least 18 years old, or have a legal guardian request services on your behalf
Provide accurate and complete information
Ensure safe and lawful use of transportation services

4. BOOKINGS AND SCHEDULING

All rides must be scheduled in advance unless otherwise confirmed.

Pickup times are estimated and may vary due to traffic, weather, or operational conditions.

The Company is not responsible for delays caused by external factors.

5. PASSENGER RESPONSIBILITIES

Code of Conduct

Passengers agree to:

Be ready at the scheduled pickup location on time
Provide correct pickup and drop-off information
Follow safety instructions from drivers
Use appropriate restraints when required (wheelchair, seatbelt, etc.)
Avoid disruptive, abusive, or unsafe behavior

We reserve the right to refuse service for safety reasons.

6. CANCELLATIONS AND NO-SHOWS

7. PAYMENT TERMS

8. SAFETY AND LIABILITY

We prioritize passenger safety at all times. Passengers must comply with safety rules and driver instructions.

The Company is not liable for delays due to weather, traffic, or third-party issues. Liability is limited to the maximum extent permitted by Florida and U.S. law.

9. NO EMERGENCY SERVICES

Healthy Heart Mobility is not an emergency transportation provider. If you are experiencing a medical emergency, call 911 immediately.

10. SERVICE LIMITATIONS

We may refuse or discontinue service if:

blockRequired information is incomplete or inaccurate
blockOperational limitations prevent safe transport

11. PRIVACY

Use of our services is also governed by our Privacy Policy, which explains how we collect and use personal information.

12. THIRD-PARTY PROVIDERS

We may work with:

13. FORCE MAJEURE

We are not responsible for delays or failures caused by events beyond our control, including:

14. MODIFICATIONS TO TERMS
